More efficient IN clauses for prefetch_related queries by use of sets to eliminate duplicates

This simply reduces the size of the IN clause in the SQL, which can be

Cleanups to related manager code, especially in use of closures.
...
The related manager classes are defined within functions, and the methods
had inconsistent and confusing usage of closures vs. parameters on self to
retrieve needed information. Everything is stored on self now.
Also some methods were not using super() where they should have been.

Make ManyToManyField model references more robust.

In r16679 a test was added to verify something had been fixed when a
unicode string type was passed in as a model name. The name has to be
ASCII convertible, but in Python 2.6 and earlier, it must also have str
type.
This commit fixes the problem for earlier Python versions and is almost
identical to a patch from b.leskes in #6045 .

Removed django.db.models.options.Options.one_to_one_field.

The last use of it was removed in r9641 (it's internal API) and it's been
broken since r7477, as there's no longer a maximum of one OneToOneField per
model, so anything relying on it contained subtle bugs.

fields no longer creates duplicate copies of the join table(s). Basically, this
means filters on the join table (for ManyToManyField(through=...)) and complex

Major refactoring of django.dispatch with an eye towards speed. The net result is that signals are up to 90% faster.

Though some attempts and backwards-compatibility were made, speed trumped compatibility. Thus, as usual, check BackwardsIncompatibleChanges for the complete list of backwards-incompatible changes.
Thanks to Jeremy Dunck and Keith Busell for the bulk of the work; some ideas from Brian Herring's previous work (refs #4561 ) were incorporated.
Documentation is, sigh, still forthcoming.
Fixes #6814 and #3951 (with the new dispatch_uid argument to connect).

Added a db_type() method to the database Field class. This is a hook for calculating the database column type for a given Field. Also converted all management.py CREATE TABLE statements to use db_type(), which made that code cleaner. The Field.get_internal_type() hook still exists, but we should consider removing it at some point, because db_type() is more general. Also added docs -- the beginnings of docs on how to create custom database Field classes. This is backwards-compatible.

Refactored workhorse methods on m2m descriptors. Modified _add to check for the type of incoming objects (to match existing _remove implementation), and modified _remove to use a single query rather than 1 per object (to match existing _add implementation).

Copied django.forms to django.oldforms and changed all code to reference django.oldforms instead of django.forms. Updated docs/forms.txt to add 'Forwards-compatibility' section that says you should not be using django.oldforms for any new projects.
