move amalgamate from bash to python (#1278)
This is much faster (from 3.5 to 0.14 seconds)

John Keiser 127d962271 Remove unneeded .gitignores
Now that cmake is the only build system, we don't generate
binaries in the source tree anymore. Amalgamate also doesn't
leave anything around.
