Bruno Oliveira 519f02b014 docs warnings as errors and initial page similar to README
* Changed "index" to be more similar to the README (#1708).
* Fixes numerous issues like missing documents, syntax errors, etc (#1829, #432).
* Mention all docs in "contents.rst" so it's easier for users to locate (#1112).
* Add doc generation and checking to Travis and AppVeyor, to avoid re-introducing errors.

Floris Bruynooghe 3df5989326 Revert pypi badge again as the svg broke
This reverts the pypi badge back to the pypip.in provided one
because the shields.io one failed to render properly (and also
wasn't on https).

Jurko Gospodnetić 9fb2079458 replace py.test module references with pytest
The only remaining 'py.test' references are:
 * those referring to the 'py.test' executable
 * those in code explicitly testing py.test/pytest module compatibility
 * those in old CHANGES documentation
 * those in documentation generated based on external data
 * those in seemingly unfinished & unmaintained Japanese documentation

Minor stylistic changes and typo corrections made to documentation next to
several applied py.test --> pytest content changes.
