New doc role: globalvar for special variables
...
This introduces a new role, `:globalvar:`, so we can mark/reference
variables like `pytest_plugins`, `pytestmark`, etc. This besides being useful
also makes the documentation look more consistent.

Remove version references to pytest 2 and 3.
Just like Python 3 no longer has references to Python 2, I think
we should follow the same approach in pytest.

Document how to disable caching rewritten .pyc files to disk
